The Community nurses provide nursing services to people of all ages in the community – working as part of the Primary health Care Team in partnership with patients, their families and other health and social care agencies.
Our home visiting team
The role of the visiting team is to triage and prioritise any requests for home visits; to offer telephone advice, support and education to staff and residents of Nursing/Care homes and to those who are housebound or too ill to travel to the surgery and family members as appropriate.
The objective of the visiting team is to visit and manage patients in the community who require a home visit; to liaise very closely with and support the duty doctor and to offer and organise contact with the duty doctor when the need is appropriate.
The visiting team plays an important role at Maywood as we have a large population of elderly patients in nursing, residential and care homes as well as those who are housebound. All members of the visiting team have received extra training including minor illness and physical assessment and are very competent practitioners.
