When Death Occurs at Home

• Telephone the surgery, a doctor will confirm death has taken place.
• Contact the funeral directors.

When Death Occurs in Hospital

• Contact the funeral directors to inform them their services will be required.
• Collect the death certificate from the hospital.

Take the certificate to the registrar for the area in which the death took place together with the deceased’s medical card and birth certificate if available.

Take the green form to the funeral director who will take over complete responsibility for arranging the funeral.
